Visit issaquahwa.gov/prr and select the type of records you would like to request. Create an account or login and complete the form when prompted.
Email or Physical Mail
Click on the City or Police form to the right in the "Attachments" box to download the form. Complete it and submit via the e-mail or mailing address listed on the form.
In Person or By Phone
If you are making a request in person or by phone, you may be asked to complete the online form or paper form. If you require assistance, please call the appropriate person listed here. They will put your request in writing based on their understanding from your conversation and request your confirmation in writing that it is correct. If confirmation is not received after 20 days, it will be deemed correct. Staff will begin processing your public record request once the request is confirmed or deemed correct.
